Before your appointment:

Please have the following information available, if applicable:

Ages and ZIP code of everyone needing coverage
Current health insurance information
Expected coverage start date
Preferred doctors, hospitals, and medications
For ACA Marketplace coverage, an estimate of your household income and household size
Information about any employer-sponsored health coverage available to you or your family

During your consultation, we'll discuss your health coverage needs and help you understand available options, including ACA Marketplace plans and short-term medical coverage when appropriate.

Please do not enter Social Security numbers, medical records, financial account information, or other sensitive personal information in the online booking notes. Any additional information needed for an application can be collected securely during the enrollment process.
