When Should I Sign Up for Medicare?

Turning 65 doesn't mean you have to figure out Medicare overnight. But knowing your enrollment dates is important because missing the right enrollment period can cause delayed coverage or, in some situations, late-enrollment penalties.

Your 7-Month Initial Enrollment Period

For most people who become eligible for Medicare at age 65, the Initial Enrollment Period (IEP) lasts 7 months:

3 months before your 65th birthday month
+ your birthday month
+ 3 months after

Example

If you turn 65 in June, your Initial Enrollment Period generally runs:

March → April → May → JUNE → July → August → September

That gives you seven months to take action.

Do I Automatically Get Medicare?

Some people are automatically enrolled in Medicare Part A and Part B, while others need to sign up.

For example, people already receiving certain Social Security benefits before turning 65 are generally automatically enrolled. If you're not automatically enrolled, you'll need to apply.

What If I'm Still Working?

This is where Medicare timing can be different.

If you or your spouse is still actively working and you have qualifying employer group health coverage, you may be able to delay Part B and enroll later through a Special Enrollment Period without a late-enrollment penalty.

Don't assume your employer coverage allows you to delay Medicare. Check with your employer or benefits administrator before making that decision.

We'll cover this in more detail in: Part 3: Still Working at 65? Do I Need Medicare?

Your Simple Medicare Enrollment Checklist

☐ Find your 7-month Initial Enrollment Period
☐ Determine whether you'll be automatically enrolled
☐ Review your current health insurance
☐ Ask your employer about Medicare if you're still working
☐ Determine when you want Medicare coverage to begin
☐ Don't wait until the last minute to understand your options

The Bottom Line

You don't have to make every Medicare decision months in advance, but you should know your enrollment window before your 65th birthday arrives.

Knowing your dates gives you time to understand your options without feeling rushed.
